Good morning, I have a claim, where CPT 93990 was denied due to patient's age. (the denial states patient's age is 0-20 year old. However, I am reading my CPT book guidelines and I do not see anything about patient's age. Patient is a 33 years old with Dx: N18.6. The duplex scan was done in the office setting. Is this is correct about the patient's age? Pt. has superior health plan (medicaid replacemnte-hmo) as his insurance. Any input, I will appreciate it. Thank you.
