The dx code that was attached to this was 490, and the LCD doesn't support this diagnosis, so my question is in regards to the documentation of the SOAP note. In the H&P section under the diagnosis of Bronchitis (490) if it states the patient has been coughing for a couple of months can I use the 786.2 for cough with the CPT code 94640 since this is a supported diagnosis code? I'm a little confused when reading the soap notes as to what we can use when billing the procedures, so any suggestions or reference material in regards to this would be greatly appreciated.
