I have a Pulmonologist that does indeed meet the criteria for CC. My issue is most insurances are not paying for the CPT 99292. They seem to be bundling it w/ the procedures (lets say CPR). Since the 99292 is an "add on code", I did not think it needed any modifier. Would I use modifier -25 on the CPT 99291 & 99292? I have never seen it done this way, but do not know why it will not pay. Does anyone have solid information how this should be done?
