Wiki CPT 99384-99397 Preventive/CPE only used annually?

Hello,
I'm looking to find something that will clarify or indicate that the preventive CPT codes 99384-99397 are used for the annual physical exam (once a calendar year) with or without the pap exam, but I can't find anything noted on the AMA CPT book. Where can I find documented that theses codes are for the annual exams or does the diagnosis Z00.0- let the payor know it's for their annual?

I’m not aware of any written coding guideline that states that these codes may only be used for‘annual’ physical exams. However almost all payers will reimburse a maximum of only one per year, and once a year is standard practice by most primary care physicians.
