Wiki CPT 99491 used in addition to E/M codes

Is anyone utilizing this code and if so does it require a modifier? Or can it be used just additionally? I know that this code is only utilized once per 30 days just wasn't sure if any additional modifiers were needed...just looking for a little more help on this ...
