CPT Category II codes - Where can I find the definition of abbreviations in parentheses after the code explanations? Some are easy to figure out. Others are not. Are the CAT II codes limited in usage to those conditions listed in the parentheses?

You can find it in the very beginning of the Chapter Specific Guidelines. I don't have my ICD-10 book in front of me, but I believe it is on page G3-G4

This is a CPT question so not sure the ICD guidelines would be useful

You have to go to the ama website and type "alphabetical clinical topics listing" in the search. It says to go to the ama site and click on the Category II link.
