Wiki CPT code 99417 for UHC

I anyone having a problem getting CPT code 99417 to UHC? Per their policy it is a valid code however their claims scrubber kicks the claim out before it makes it to adjudication when the CPT code is on the claim saying it is not an appropriate code when billed with office visit E &M code.

UHC has been following Medicare guidelines. Check the Prolonged Services policy with UHC, but I think they accept G2212 instead.
