I am seeking coding advice for the following scenario:
Patient with prior (10+ years) surgical mitral valve repair and left atrial appendage ligation presents for percutaneous LAA closure due to incomplete ligation/leak left atrial appendage. A vascular plug/coil is used for the closure of the incomplete ligation, as opposed to a Watchman or Amulet (two FDA approved devices for LAAC).
May CPT 33340 (Percutaneous transcatheter closure of the left atrial appendage with endocardial implant...) be coded for the procedure described above?
The CPT 33340 description states "closure" of the LAA, which a vascular plug/coil achieves; however, the CPT 33340 description also specifies "...with endocardial implant." Does the plug/coil satisfy the specification of "endocardial implant"?
Should the Unlisted CPT 33999 be used instead?
