CPT code for Physcican 45390 coding

Patient came in had multiple polyps removed by snare and biopsy throughout the colon. Doctor is coding 45390, 45385 w/59 modifier, and 45380 w/59 modifier, however researching 45390 CMS is not accepting for physician charges. How do I go about coding this? I researched and 45390 is in the cross walk of 45378 and 44799, please advise this coding for new CPT code.

Did the physician inject the lesion and then snare it?

my book says do not report 45390 with 45.80-45381,45385,[45398] when on same lesion, and not with 45378

One polyp was removed with a saline injection lift using hot snare.
Another polyp he removed only by snare, then removed three other separate polyps using cold forceps.
