Dr did a tympanoplasty and mastoidectomy. I just can figure out the OCR and
The bone was mildly sclerotic, Antrum was opened after the tegmen was identified. sinodural angle identified sigmoid sidnus and posterior can wall was thinned. with the opening into the antrum cholesteatoma was identified. Incus was free floaitn w.o connection to the stapes. Incus removed.
Malleus nippe was used to remove the head of malleus. a facial recess was then drilled
a temporalis fascia graft was harvested. w. silastic sheet cut to shape and places. facial graft was laid medial to handle the of the malleus and the tympanomeatal flap raised.
