Need help with a CPT code for manually breaking down penile adhesions in a child greater than 28 days. One of my physicians had been coding the 54162 for lysis of postcircumcision adhesions but the description states that it is done under general anesthesia. This has been done in the clinic, without incisions and without anesthesia. I am at a loss on what to code because the other possible code 54450 states "uncircumsised". Any help would greatly be appreciated! Thanks
 
Recommend code 54450 as the CPT description is foreskin manipulation including lysis of preputial adhesions and stretching. There is nothing in the code descriptor regarding uncircumcised.
 
54450 cdr states:The physician treats adhesions between the uncircumcised foreskin and the head of the penis that prevent the retraction of the foreskin. Adhesions are broken by stretching the foreskin back over the head of the penis onto the shaft or by inserting a clamp between the foreskin and the head of the penis and spreading the jaws of the clamp.

A later thread states manually breaking down penile adhesions in a child greater than 28 days for a circumcised pt is inclusive to the e/m.
