The physician excises a fissure (fissurectomy) and may perform a sphincterotomy. The physician identifies the anal fissure and the internal sphincter muscle by palpation. An incision is made around the fissure, dissecting it free of underlying sphincter muscle, and the fissure is excised. The internal sphincter muscle may be incised, usually in a lateral position away from the fissure. The incision is usually left open to allow drainage.
