So we are trying to precert this surgery, so no Op note yet.
Patient has a septal mass near level of inferior turbinate. This will be removed endoscopically and:

This may require septoplasty for access and he may require a mucosal graft from the inferior turbinate.
 
You need to speak to the doctor. This may involve a inferior turbinectomy (either 30802 via submucosal coblation or 30140 via submucosal resection), it may involve a Septoplasty, 30520 and it may require a cartilage graft from the septum, 20912. I would precept every possible code that might be done and you can always call the insurance after the surgery and correct what was really needed once the doctor started doing the surgery.
