I am trying to code a reattachment of a partial amputation of the left thumb. Notes states patient sustained a degloving/amputation of the left thumb nail and the pulp of the thumb. The area was cleaned and irrigated. The fingertip itself was prepped and draped in the usual sterile fashion and the amputated piece was reapplied and reapproximated with sutures. The nail was tucked underneath the cuticle. Any help would be appreciated.
 
As per CPT guidelines, "the CPT code 20827 - Replantation, thumb (includes distal tip to MP joint), complete amputation.
For repair of incomplete amputation of digit, thumb, report the specific code for repair of bone, ligament, tendon, nerve and blood vessel and append modifier 51, 59 as appropriate. "

For repair in general, consider 12041. Unfortunately, the notes do not specify tissues involved in the repair to justify the service.
