CPT / HCPCS code for PCP deciding to refer a patient to ER due to an urgent problem

Hi,

Is there a reporting code or CPT code if a PCP sees a patient in the clinic for a problem and the PCP decides that the patient should go to ER based on his evaluation?
 
It may factor into your level of MDM, but there is no separate procedure code to report that you referred a pt to ER (or any other physician).
