I need some help with the following Op note please:

DX: 1) S/P MVR / CABG
2) Sternal Nonunion

Procedure: 1) Bilateral pectoralis major flaps
2) Sternal Plating

I think the CPT for Flaps is 15734 however I am having trouble with the sternal plating code.

Any help will be greatly appreciated.
 
Look at CPT 21825 - CPT code description does not specify that if has to be a traumatic fracture. In my opinion, if I'm understanding your documentation, the nonunion is result of therapeutic fracture for MVR/CABG procedure. This code is separately billable with 15734 per NCCI edits. As a last resort you could use the unlisted 21899.
