preop and postop diagnosis: none

procedure performed previously: CPT 33405- Aortic valve replacement.

description of surgery: the chest was prepped and draped in usual fashion. the old midline incision was reopened. the old sternal wires were cut and removed. the mediastinal blood clots were evacuated with copious irrigation of the pericardial cavity. all the suture lines were thoroughly evaluated and there was no obvious bleeding from any cannulation sites. there was oozing from the chest wall which was treated with cautery from previously placed sternal wires. otherwise there was no active bleeding. this simply was a diffuse coagulopathy. floseal and surgicel were repositioned along the aorta as prophylaxis. once all the clots have been removed and hemostasis was ensured , closure commenced. the chest tube were irrigated and repositioned . the sternum was reapproximated with #7 stainless steel wires. the midline was closed with interrupted 2 ethibond sutures. two layers of 2-0 vicryl was used to close the subcu tissue follwed by 4-0 monocryl for the skin. the patient was transferred to the ICU in stable condition.

I think you are correct, 35820-78 would be more accurate if this is a post-operative complication. The note doesn't specifically state that is what prompted the surgery, but in re-reading it does seem apparent that is likely the case here.
