I am trying to rationalize what CPT to use for excision of left breast mass in the superior region around 12 o'clock position that was evaluated with ultrasound and showed lipoma about 4 x 6 cm in size. The lipomatous mass was identified and carefully pulled into the wound.
 
I'd be looking at CPT 19120, and you can add CPT 76998 only if permanent images are obtained and saved,and an ultrasound report is entered into the medical record.
