CPTs 22864 & 22554 together

Dear All:

Can we bill CPTs 22864, 22554 & 22851 together when removal of artificial disc , arthrodesis & cage insertion done on a cervical spine level C5-6 .My doubt is whether CPT 22864 alone is sufficient?

Your kind advice is highly appreciated!

this is all I can find, but I am now thinking it is old
Excerpt from an Ortho Pink Sheet...(date 5-3-10)

Revision of cervical artificial disc (22861) includes components 22220, 22505, 62291, cervical fusion (22554 and 22600) and spinal fusion (22800, 22802, 22808 and 22810), as well as 22856.

Removal of cervical artificial disc (22864) includes components 22220, 22505, 22554, 22600, 22800, 22808, 22845, instrumentation (22851), 22856, 62291, and diskectomy (63075).

I checked the NCCI edits also and 22856, 22861, 22864 are bundled into 22554
