When a new PA or NP starts with a group and they are credentialed with insurance companies, do you also need to file a letter with the insurance company stating they will be working under and filing claims under a particular doctor's NPI?

From my experience, that information is typically part of the application - covering providers, etc. I never filed an additional letter of explanation.
Most carriers follow the Medicare guidelines regarding incident to, so you would only submit the claim under the physician if the visit meets those requirements.
