I am receiving rejections when billing for critical care (99291) along with thoracentesis (32421) which were performed the same day. I have tried using modifier 25 on the critical care. When calling I was told there was not modifier needed but it is being rejected against each other. Any thoughts?
 
Critical Care and thoracocentesis

Did you take into account the Time factor to report the total duration of time spent by the physican providing critcal care devotedservice and worked it out on the code Table formula: 99291, once, and the ADD ON CODE for each additional 30minutes (with the code number 99292), apart from your procedure code for thoracocentesis), which is ofcourse not included in Critical Care service reporting.
