Anyone know if there is such a crosswalk/tool available and where?
 
Pretty sure Ingenix has one. Most encoder programs also "crosswalk."

Word of caution: the crosswalks aren't always correct and you must keep in mind that CPT and ICD have different standards and guidelines in procedural coding.
