how would you code the following:

Current CRT-D (RV, RA, LV, and device)

procedure: RV lead extracted, new RV lead implanted, device replaced (CRT-D can out; new CRT-D can in)

RV out 33244

RV in with a CRT-D gen change : 33249 for the new RV lead as well as the generator ?

ICD generator out: 33241
