The physician dictates that he performed neurolysis of segmental nerve root via cryoablation(two distinct lesions), cryoablation of greater and lesser occipital nerves and cryoablation of the right C2 and C3 facet nerve brances.
Would the segmental nerve roots be inclusive to the facet nerve cryoablations, or would they be reported with 64640? Any help is greatly appreciated!!
