Does anyone know if a CT lung screening diagnosis (referral) code can be piggybacked with a regular Dr. visit's E & M codes? In other words, so the patient wouldn't have to come back for a separate encounter in order to get the order for the screening, can they come in with a cough (hypothetically) & have a history of smoking & their PCP just put the code required for them to get a lung screening CT on that same day's encounter with the corresponding E & M codes that would normally be submitted?
Any help is greatly appreciated!

If I am reading your post correctly, does the physician not have to provide the hospital with an H/P if the patient is having a procedure performed or at least document the reason for encounter/admission when the procedure is performed? If so, you can use any diagnosis listed by the provider on the H/P or the surgical report to support your diagnosis.
