Documentation states: History of CVA with Right Paresis. I also have documentation of Muscle Weakness. Wouldn't the right Paresis be the Muscle Weakness and I would use the codes: 438.89 for the CVA and 728.87 for the Paresis/Muscle Weakness? Any help would be greatly appreciated.
 
CVA with right paresis

I would use 438.2x Late effects of cerebrovascular disease with hemiplegia (paralysis of one side of the body). Fifth digit depends on whether the physicain documented the patient's dominant side or not.
