Please help how would you code the attached report

Following informed consent using sterile preparation and 1% lidocaine for
local anesthesia an AccuStick needle was introduced into the liver under
fluoroscopic guidance. On the second pass, a peripheral branch of the
right hepatic vein was entered. A wire was advanced and a sheath was
placed. Hepatic venogram was performed demonstrating this may be widely
patent. A 5-French double lumen PICC catheter was placed with its tip
overlying the IVC right atrium junction as verified by fluoroscopy.
Total PICC length is 20 cm. According to the radiation monitor the dose
area product is 1305 cgycm2. Approximately 10 cc Visipaque 320 contrast
material was used. . Patient tolerated procedure well. Images and report
were permanently recorded.


Impression: Successful transhepatic PICC placement.
