I have an operative report for procedure: cystoscopy, right retrograde pyelogram, right flexible ureteroscopy, biopsy of ureter with piranha forceps....

I am new to coding for urology, can someone steer me in the right direction..

I am having trouble getting 93281 paid due to medically necessity... the dx codes used are Z95810 and I492.... should I be using something else?
