Hello,

I would like to know which documentation is it appropriate to use when selecting ICD-10 codes for ASA coding for profee coding, the provider's op note, the ASA record, the pre/post procedure evaluation notes?


We have been using the provider op note as the primary source for selecting our ICD-10 codes. On some recent audits of my team the auditor has been stating to use the ASA provider records also to pick up addition diagnosis codes.
