If 2 diabetic ulcers are debrided both to full thickness, would 11041 have not only modifier 59 on it but 2 units declared also?

It depends on your insurance, but I usually will bill them out separate line with 59 mod.

This question has been asked many times on this forum and gets many answers. You might try a search for debridements, I bet you will get a boat load of information.
