I need a little bit of help. I am unsure of how to code the following op report:
"The patient had a large 10 x 10 cm, necrotic, deep sacral decubitus. A skin incision was made with a 10-blade scalpel, excising the necrotic eschar. There was foul-smelling pus beneath the eschar, and there was necrotic soft tissue extending down to the sacrum and extended laterally on either side. Additional skin, subcutaneous tissue, and muscle were debrided laterally on either side down to viable tissue. This left the wound about 20 x 20 cm. There was significant bleeding from the soft tissue, and this was controlled with electrocautery and figure-of-eight 3-0 Vicryl sutrures. Eventually, hemostasis appeared to be secure. All the necrotic tissue had been debrided."
My physician is trying to code it as 11042 and 11045. I do not think this is correct.
Any help would be greatly appreciated! Thank you in advance for your help.
