Definition of Wounds, Lacerations, Puncture?

Is there somewhere I can find that defines the use of open wounds, lacerations or puncture wounds in ICD-10? We get a lot of skin tears in our facility, and I am not sure if I should use unspecified open wound or laceration?

These are elderly patients that fall or bump into things and their skin just tears. I am trying to do the wound portion
