just wanted to check myself here because I am light on documentation. I am looking for both ICD9 and CPT codes, if possible.

.....Patient comes in for outpatient services. Physician order states low back pain and lumbar radiculopathy. X-ray of the lumbar spine shows degenerative disk disease of the lumbar spine with spondylosis.

What are my ICD9s?....this is not in the:) Radiology dept, so I am not even sure if I can code the Xray cpt. Please help.:)

One of my Icd9s is 722.52....
