Denial 33361

Hello,

I have been billing 33361 with modifier 62 and have been getting denials for inconsistent modifier or modifier missing for Medicare and other Medicare Advantages. Is there other modifiers that I am missing?

I bill 33361-62-Q0 with dx I35.0 & secondary dx Z00.6 and add the NCT# to the claim.
