Wiki denial code 16

I need help. New at coding for the psyc area. I have received a denial code 16 on this one patient and am lost as to what they are wanting. I my be just over looking and don't see it for the trees. LOL. Claim/Service lacks information which is needed for adjudication. Please help if any one has had this problem.

You need to call the insurance carrier - they either need info from the patient or notes from the provider.

Thanks for responding. It's medicare with they be more helpfull than usual? I hope so.
