Aetna has been denying our claims for PEG (CPT 43246) w/ modifier 62 stating that multiple surgeons is not allowed for this CPT. I'm trying to draft an appeal letter to accompany op notes from both surgeons. Does anyone has an example I could follow or tips of what to include?

43246 does allow for a mod 62. aetna might require the surgeons to be of different specialties, gastro and gen surgery for example.
