My mom got a medicare procedure denial letter for procedure 86021-9004. I can find 86021 but not the combination 86021-9004. What is this and why was it denied?
 
I'm not exactly sure. I'd contact the lab for clarification. It could be an internal code used for indentification or reporting purposes, or it could be and ICD-9 procedure code. 90.04 is the code for Microscopic examination of specimen from nervous system and of spinal fluid for parasitology.
