Denial of code 93458 by IEHP & Medical

Please help we are getting denials for code 93458 by IEHP and Medical? Any suggestions? Also, can anyone tell me what the modifers are for IEHP and Medical for the heart cath? We have been using 26 only for the 93458? We are also having trouble billing nuc stress test for Medical, I know there is a different set of codes and modifiers and I have gone as far as contacting Medical with no success???? :confused:
