A Z code for "presence of tubes in ear" came back from insurance. They do not accept this diagnosis for 69424
That is the only diagnosis documented.
May I query the surgeon to do an amendment/addendum to add a more specific dx, such as chronic otitis media when he is doing surgery to remove the indwelling tubes previously place by another MD?

What Z code was billed?

Z45.82 is "Encounter for adjustment or removal of myringotomy device (stent) (tube)" - that should be payable for a tube removal.

Or did you bill Z96.22 "Myringotomy tube(s) status" instead of the encounter code for removal?
 
Also, was the procedure performed under general anesthesia?

I just wanted to confirm that CPT 69424 was the correct code choice as well.
