I am getting Medicare claims that are being denied for Active reason 50, M25 or N115: due to LCD, however, the diagnosis is listed on the LCD/Article. This is not an authorization issue or a Place of Service issue. What other factors could be in play here?

What is the service and what are the diagnosis codes?
