Wiki denials for payment of G0482 due to lack of medical documentation support?

I work w/ an independent lab that performs drug testing. Lately, I'm seeing more denials due to lack of medically documented support for the number of test performed (G0482 = 14 to 21). We're sending a copy of the signed order, and the results. Anyone run into this issue? What are you doing to retrieve payment?
