They haven't been "deleted"; rather, the ADA no longer allows publication of their codes in the HCPCS manuals or in on-line coding products without a separate licensing fee. You can contact the American Dental Association directly and order their Dental Procedure Manual (you can order it on-line from them); if you use a product such as Encoder Pro or Code It Right Online, contact your representative to have the codes added; you will likely have to pay an additional fee.
