I have a depo injection that the info is filled out on a pre-made form. the information is relative to injection only. on the bottom of the form is 2 sentences that state the pt wants to continue depo and add back estrogen. doesnt want patch and follow up at next depo appt. Now am i missing something or does this meet, if so, how? thanks
 
If the visit was schedule as a depo injection encounter then you may bill for the injection only. If this is a physician encounter to determine if the therapy is correct etc, then I think the documentation is insufficient.
