Wiki Depth for biopsy code 27614

I have a code for a punch biopsy of a skin lesion. Path report says it measured .6 x .6 cm with a depth of .2 cm. How do i know if this should be code 27613 or 27614? There is no mention in the procedure note of superficial, subfascia or intermuscular. Thank you in advance.
 
Both 27613 and 27614 are for the soft tissue biopsies, not for skin lesions. For 27613, the dissection is carried down usually through the subcutaneous fat to the uppermost fascia layer, while for 27614, it is deep within the soft tissue, within the fascia or muscle.

For a punch biopsy of a skin lesion, I would use 11100.
