When billing for a Dermagraft application, is it correct to bill both G0440 and G0441 per each 38 sq cm sheet of product used? Even if the actual size of the ulcer is is under 25 sq cm? Or do you code according to how many sq cm of the sheet was used?

You should bill 38 units per each sheet used regardles of wound size, G0440 would be billed based on wound size and not the size of Dermagraft sheet. So for your case with ulcer measuring less than 25sq cm you should bill:

G0440 + Q4106x38 + any casting and so on.
