This claim is for 11301 biopsy shave, 17000, 17003. We tried modifyer 59 on the 17000. Got rejected. Tried 51 on the 17000. Got rejected. It's got to have a modifyer somewhere! Help. How would you code this? Thanks!!!:confused:
 
You need to check the CCI edits to see which code is considered the comprehensive and which is the component. the 59 modifier goes on the component code. Also the documentation must support that these were separate lesions and not the same one.
