Wiki Destruction versus Biopsy

I have an insurance company stating that when a destruction (17000) is billed with a biopsy (11100 & 11101) that we need to put a 59 modifier on all three cpt codes. They state this is the correct guideline that they found in the Principles of CPT COding. Since when is this appropriate? Thanks for your help as always
 
I just put the 59 modifier on the 11100 and have never had a problem billing it this way. I also double checked this against the software I have for checking CCI edits (Inga Ellzey's dermcoder) and it is also showing the 11100 should be the only one getting the 59. :)

We have this problem with one of our private insurance companies. The created their own version of the CCI and most everything gets a 59 modifer.
