Please advise how to bill the correct units for Dexamethasone Sodium Phosphate 6mg. The provider order 6 mg and as per documentation they use 2 vial of Dexamethasone 4 mg/ml

I have searched and found a lot of information on the internet but it was not clear to me and I always had doubts. Could you please tell me about the JW modifier; Does it only apply to Medicare patients or also to Medicaid and FFS lines of service? Once again; thank you for you help🙏
