Patient has poorly controlled type 2 DM and end stage CKD, which the patient is receiving dialysis for. How is this coded? Is there a presumed link between DM and CKD?
 
As of Oct 1 2015 there is a presumed link between DM and CKD. When you use the alpha index of the code set you look under diabetes, immediately under this you see the word "with" everything listed under the word with is a presumbed causal relationship.
 
yes and if documented as poorly controlled you add the e11.65 for hyperglycemia. Again per the index under diabetes, the poorly controlled tells you to code to diabetes with hyperglycemia.
