Can anyone tell me if CMS ruled on using " with" when coding diabetes with complications. ex.- diabetes with nephropathy. :confused:
 
You code the diabetes code first with the 4th digit indicating complication and then the code for the manifestation. The diabetes code can be either 250.xx or 249.xx depending the type supported by the documentation. You might want to clarify that the nephrology is a complication of the diabetes.

Diabetes WITH Nephropathy

Code is 250.4X (add 5th digit), and 583.81. As last person said, confirm that the nephropathy is a manifestation of the diabetes with the physician (who should have documented this).
